AMATEUR ATHLETIC RELEASE OF LIABILITY

In consideration of being allowed to participate in any way in the Edmonton Columbians athletic sports program, related events and activities, I ____________________________, the undersigned acknowledges, appreciates, and agrees that:

1. The risk of serious and potentially fatal injury from participation in activities involved in this program is significant

2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known/foreseen and unknown/unforeseen.

3. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS The Edmonton Columbians Track and Field Club, their officers, officials, agents and/or employees, coaches,

volunteers, other participants, sponsoring agencies, sponsors, advertisers and in addition, any owners and lessors of premises used to conduct the event (“Releasees”), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or

damage of whatever kind to my person or property, howsoever caused, including but not limited to THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE.

I HAVE READ THIS RELEASE AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT BY SIGNING IT, I HAVE GIVEN UP SUBSTANTIAL RIGHTS, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

FOR PARTICIPANTS OF MINORITY AGE (UNDER AGE 18 AT TIME OF REGISTRATION)

This is to certify that I, as parent/guardian with legal responsibility for the participant, do consent and agree to his/her release as provided above of all the Releasees, and, for myself, my heirs, assigns, and next of kin, I release and agree to indemnify the Releasees

from any and all liabilities incident of my minor child’s involvement or participation in these programs as provided above.

Travel and Medical Authorization

The Edmonton Columbians Track and Field Club may choose to compete in track meets outside the Edmonton area during the membership year. This is optional for the athletes. All club travel events are to be pre-approved by the executive. During travel, the

Edmonton Columbians/Athletics Alberta code(s) of conduct will be adhered to or the athlete may be sent home at the expense of the athlete or parent/guardian. I agree to adhere to The Edmonton Columbians Track and Field Club Travel Policies when travelling

representing the Edmonton Columbians. Athletes under 18 years of age will require a parent/guardian to attend all out of town meets.

Travel outside of Canada: For out of country travel, it is mandatory for athletes to obtain out of country medical coverage. Proof of coverage will be required prior to ANY travel outside the country. Athletes must have passports for all foreign travel.

Emergency Treatment: Emergency medical or dental care may be required if an accident or illness occurs during participation in training, events or travelling with the Edmonton Columbians Track and Field Club. Attempts will be made to contact the parents/guardians or emergency contacts. I authorize Edmonton Columbians Track and Field Club representatives to grant emergency medical, dental or hospital care to the above named athlete. I understand that I will be financially responsible for any such care rendered on behalf of the athlete.

Agreement:

I have read and agree to abide by the information included in Membership Information for the Edmonton Columbians Track and Field Club. I have read, completed and agreed to the Amateur Athletic Release of Liability and the Travel and Medical Authorization Form.

As an athlete (or parent/guardian of an athlete under age 18), I agree to allow the Edmonton Columbians to use my name or likeness within their website or in other circumstances pertaining to the club’s activities. This includes media coverage which may include photographs, videos or other media. I agree to allow the use of registration information for club purposes.
