ICKF Karate School

I, hereby make application for membership in International Chito Ryu Karate & Fitness Inc. Upon acceptance I sincerely pledge to obey all club rules and regulations, which were formulated for the purpose of keeping order in the club and for the protection of pupils from injury. I further acknowledge and understand that a risk of personal injury is involved that requires my strict adherence to the rules and regulations and to the instructors discipline.

In consideration of accepting my position and a further consideration of the amount paid, which said sum represents a year membership in the Canadian Chito Ryu Karate Do Association, fee (receipt of which is hereinafter acknowledged by International Chito Ryu Karate & Fitness Inc.) and in further consideration of the three month, or yearly, fees required by the rules and regulations for the participation in the club’s activities. I, my heirs, executors and administrators do hereby forever release and discharge 2006467 Ontario Inc. and International Chito Ryu Karate & Fitness Inc., its President, Instructors, members and authorized guests from all responsibilities and all claims for injury which I may receive while practicing Karate; and the parent or guardian of the Applicant hereby requests that this application be accepted, And in consideration of this acceptance and the monies to be paid aforesaid, hereby agrees to indemnify 2006467 Ontario Inc. and International Chito Ryu Karate & Fitness Inc. , its President, Instructors, members and Authorized guests, of and from all manner of claims made by or on behalf of the Applicant.

I have read and understand all of the aforementioned rules and regulations and sincerely pledge to adhere to them and the Instructors discipline.
